People Caring For And About People. Your Role as an MDS Nurse:

  • Conduct initial and ongoing MDS assessments in accordance with federal, state, and facility regulations
  • Accurately complete all sections of the MDS, including RAI and care planning documentation
  • Collaborate with the interdisciplinary team to ensure accurate documentation of resident needs and goals
  • Provide education and guidance to staff on MDS requirements and regulatory standards
  • Ensure compliance with all federal and state regulations related to long-term care
  • Participate in quality improvement initiatives and regulatory survey processes
  • Maintain accurate, up-to-date records of MDS assessments and care plans
  • Coordinate timely transmission of MDS data to appropriate agencies
  • Participate in interdisciplinary care plan meetings and provide clinical input
  • Monitor and update care plans based on changes in resident condition
  • Maintain case-mix index at target levels
  • Communicate effectively with physicians, staff, residents, families, and vendors
  • Follow all facility policies, including infection control, safety, and HIPAA regulations
  • Maintain a calm, supportive, and professional environment
  • Attend required trainings, in-services, and meetings
What We're Looking For:
  • Graduate of an accredited School of Nursing
  • Current Ohio LPN or RN license
  • Current CPR certification
  • Minimum of 1 year of experience as an MDS Nurse required
  • 3+ years of experience in long-term care or skilled nursing preferred
  • Strong knowledge of federal and state long-term care regulations
  • Excellent attention to detail and accuracy
  • Strong organizational and time-management skills
  • Effective communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Proficiency in Microsoft Office and electronic medical records (PointClickCare preferred)
  • Physically able to meet job requirements, including lifting and assisting residents as needed
